The goal of the pituitary tumor surgery is restoration of the normal endocrine function and releaving the pressure effects of the tumor mass on the adjacent neural structures. The authors had proceeded with the 10 years prospective follow-up study for the endocrine function and recurrence of pituitary tumor in the patients who received the complete total resection of tumor mass by the means of total capsulectomy.The authors will discuss the preliminary result of 2.5 years follow-up of this study.

Evaluation of the Usefulness of Free T4 Measured by 2 - Step Iimmunoextraction in the Patients with Thyroid and Non - Thyroid Diseases.

Many methods and commercial kits have become available for directly measuring free thyroxine(free T_4). However, most of them are inadequate for routine laboratory use because of their complexity and inaccuracy. Recently, 2-step RIA methods by immunoextraction(2-step immunoextraction) for free T_4 have been developed to improve its accuracy and convenience. We evaluated the usefulness of free T_4 measured by 2-step immunoextraction compared to free T_4 index(FT_4I), free T_4 measured by 1-step RIA, and TSH in 204 patients with thyroid and non-thyroid disease. There were no differences in sensitivities and specificities of free T_4(1-step, 2-step), FT_4I and TSH in the patients with hyperthyroidism and euthyroid nodule. However, the sensitivity of TSH in hypothyroidism was remarkablely higher than the others(100.0% vs. 61.5-81.5%). The sensitivity of free T_4(2-step) was also higher than those of free T_4(1-step), FT_4I and TSH in the patients with non-thyroid disease(94.3% vs. 74.3-83.8%). The values of free T_4 measured by 1-step RIA were significantly lower than those by 2-step immunoextraction, especially in the patients with non-thyroid disease(1.10+-0.04 vs. 1.55+-0.03ng/dL, p<0.05).In conclusion, no significant differences were found in sensitivity and specificity of free T_4(1-step, 2-step), FT_4I, and TSH in the patients with thyroid diseases except in hypothyroidism. However, the sensitivity of free T_4 measured by 2-step immunoextraction was significantly higher than the other tests, especially in the patients with non-thyroid disease. Therefore, free T_4 assay using 2-step immunoextraction is useful to differentiate the non-thyroid disease from thyroid diseases.

The aim of this study was to evaluate the quantitative changes of apoprotein levels in non-insulin dependent diabetics.As the subjects of this study, 37 patients were selected among non-insulin dependent diabetics admitted to Dong-A University Hospital between March 1990 and February 1994. They were divided into two groups. One group(n=20) had the diabetic dyslipidemia(Group DD) and the other group(n=17) had no diabetic dyslipidemia(Group D). For the control, normal healthy persons(n=17) were studied.Plasma lipid and apoprotein levels of all the subjects were measured. The results were as follows;1) Apoprotein AI & AII levels were significantly decreased in Group DD compared with both Group D and Control, but there was no significant difference between Group D and Control.2) Apoprotein B levels were significantly decreased in both Group DD and Group D compared with Control, but there was no significant difference between Group DD and Group D. 3) Apoprotein CII levels were significantly increased in both Group DD and Group D compared with Control, but there was no significant difference between Group DD and Group D. 4) Apoprotein CIII levels were significantly increased in Group D compared with both Group DD and Control, but there was no significant difference between Group DD and Control. 5) Apoprotein E levels were not significantly different among three groups.These results suggest that NIDDM itself causes the level of apoprotein B to decrease and the level of apoprotein CII to increase, regardless of the associated diabetic dyslipidemia.

It is aim of the present study to gain more insight into the role K_ATP-channel in acetylcholine(Ach) and norepinephrine(NE) release in the central nervous system. Slices from the rat hippocampus were equilibrated with [^3H]-choline and [^3H]-norepinephrine and the release of the labelled products was evoked by electrical stimulation(3 Hz, 2 ms, 5 VCm^-1, rectangular pulses, 2 min), and the influence of K_ATP-channel openers and other related drugs on the evoked tritium-outflow was investigated.Cromakalim, in concentrations ranging from 10 to 300 uM, decreased the evoked [^3H]-Ach & [^3H]-NE release in a dose-dependent manner without changing the basal rate of release. Diazoxide(10-300uM) inhibited the evoked[^3H]-NE release but increased the evoked [^3H]-Ach release significantly, while, the effects of pinacidil(3-100uM) were vice versa. Glibenclamide(1-30uM), a selective K_ATP-channel blocker, did not affect the evoked or basal release of both tritium outflow. And the inhibitory effect of K^+-channel openers was inhibited by glibenclamide pretreatment. Pinacidil-induced increase in the evoked NE release was not affected by cocaine, but potentiated by hydrocortisone. And the diazoxide-induced increase in Ach release was not affected by hydrocortisone pretreatment.These results suggest that ATP-sensitive K^+ -channels are present in the both adrenergic and cholinergic nerve endings of rat hippocampus, but these channels seem to be pharmacologically different from those reported for smooth muscle cells and pancreatic beta-cells.

Amyloidosis is a systemic disorder of unknown etiology, The pathophysiological precess is caused by deposition of amyloid fibrils in tissues of the body. Almost any organ in the body can be affected, but amyloid goiter is a very rare presentation.A definitive diagnosis of amyloidosis requires tissue examination using special stains and electron microscopy. We had recently experienced a case of 62 year-old man who was found to have amyloid goiter after thyroid surgery.The patient was admitted Keimyung University Medical Center for evaluation of rapid growing thyroid mass and lobectomy of right thyroid gland was performed.The light microscopic finding showed eosinophilic hyalinized stroma containing one thick-walled blood wessel in the hematoxyline and eosin stain. Congo-red stain on polarized light showed apple-green birefringence of amyloid in thyroid and abdominal fat tissues. Using the PAP method, nodular deposits of amyloid in the stroma and blood vessels are stained positively with anti-AA serum. Electron microscopic finding showed randomly arranged and nonbranching fibrillary rods in the stroma.

Acute thyroiditis(acute suppurative thyroiditis, bacterial thyroiditis, pyogenic thyroiditis) is an uncommon inflammatory disease, usually bacterial in orign, but may be fungi, parasitic organisms. A case of acute suppurative thyroiditis was reported and review of literature was made. A 51-year-old female noted pain and swelling of her neck. Cardinal physical finding on admission was enlargement of thyroid gland accompanied by tenderness and local heat. Diagnosis of acute suppurative thyroiditis was made by needle aspiration of the thyroid gland. The aspirated material showed inflammatory cells and follicular cells. In culture, Xanthomonas maltophilia was isolated. After antibiotics administration, she was discharged with improvement of tender enlargement of the thyroid gland.

Nesidioblastosis, the process of differentiation of pancreatic islets from ductular epithelium, is a frequent cause of hyperinsulinemic hypoglycemia in neonates and infants, but rare in adults. A 38-year-old man had suffered from intermittent hypoglycemic symtoms for 10 years. He had inappropriately elevated plasma immunoreactive insulin level and I/G ratio during a prolonged fast. There was no evidence of tumor on the abdominal CT scan and operation although celiac angiography showed a round mass like lesion in the distal pancreatic area.His hypoglycemic symptoms were improved by distal pancreatectomy.Pathologic finding in the resected pancreas was that of focal type nesidioblastosis I.e, increased size and number of islets with scattered numerous small islets around ductules in focal lesion. Distal pancreatectomy as an initial operation may be effective in the treatment of focal type nesidioblastosis.

Polyglandular autoimmune(PGA) syndrome designates the dysfunction of endocrine and nonendocrine system involving two or more glands on the basis of autoimmunity. There are three types of PGA syndrome and their pathogenesis is still not completely understood. A 27-year-old woman developed polyglandular autoimmune syndrome manifesting insulin-dependent diabetes mellitus, myasthenia gravis and Graves' disease associated papillary thyroid carcinoma. The thyroid antimicrosomal antibody and antiacetylcholine receptor antibody were positive. Her HLA serotype was A2, A11, A62, B60, CW3, CW4, DR4, DR9.We report here a case of polyglandular autoimmune syndrome, type III manifesting insulin-dependent diabetes mellitus, myasthenia gravis and Graves' disease associated with papillary thyroid carcinoma.

Recent reports have fueled on interest in the prevalance and significance of metastatic calcium deposition in patients with chronic hyperparathyroidism. Nephrocarcinosis and cutaneous cacifications have been recognized in patients with chronic renal failure, but the hypercalcemic crisis and systemic calcinosis are extremely rare in patients with primary hyperparathyroidism. We experienced a patient with primary hyperparathyroidism, presented with hypercalcemic crisis and metastatic calcification in the lungs, stomach, liver, and disappeared by successful parathyroidectomy.

thyrotropin-secreting(TSH) pituitary adenoma is extremely rare condition causing hyperthyroidism by uncontrolled secretion of thyrotropin. Among total pituitary adenomas, less than 1 percent are thyrotropin-secreting type, and only about 150 cases have been reported till now. However, increasing numbers of cases are being found due to the availability of ultra-sensitive TSH assay. We report here a patient with thyrotropin-secreting pituitay adenoma secreting TSH with production of lutenizing hormone(LH), follicle stimulating hormone(FSH), growth hormone(GH), prolactin(PRL) and free alpha-subunit.A 21-years old man visited thyroid clinic because of palpitation and dyspnea on exertion. He had diffuse goiter and features of mild thyrotoxicosis, and his thyroid function test showed increased T3, T4 and normal TSH. Serum free alpha-subunit concentration was 7420.5pg/ml(24.7mIU/ml) and the molar ratio of free alpha-subunit to TSH was 15.9. Basal pituitary hormone levels except TSH and the response to combined pituitary stimulation test were normal. A large sellar mass extending into frontal lobe was found on sellar MRI, so it was removed surgically leaving residual tumor mass due to the extensive nature of tumor. In immunohistochemical study the tumor tissue was stained with antibodies to TSH, LH, FSH, GH and PRL. His hyperthyroidism and goiter disappeared after partial removal of tumor. Somatostatin analogue(octreotide acetate) were given continuously via subcutaneous route(150 ug/day) using infusion pump to reduce the size of residual tumor. After 4 months of octreotide infusion, the size of tumor decreased slightly, and he is under treatment without any side effect.
